Cannot format disk / likely broken disk?

I am trying to format a disk, which is connected as unassigned device. Unfortunately, formatting always "fails".

 

Maybe the disk is broken?

 

I am attaching diagnostic log.

    Disk appears to be failing, you can confirm by running an extended SMART test.

    Thanks. That's what I suspected. I don't think I can run SMART as I cannot spin up the disk. Seems like a clear case of "failure"... 😞
