[SOLVED] Disk Showing as Disabled w No Errors being reported

About a day or 2 ago one of my disk started showing as disabled in the array and when i look at the disk log no errors are showing.  I've restarted array, restarted the host, unplug and replugged the hard drive and still its coming up a disabled.  The diagonstic log for the disk is below:
