Disk 4 in error state

Hello last night I was checking unraid and noticed that Disk 4 was not showing a temperature reading. Just now I remembered this and checked it again and noticed that the main section showed Disk 4 as not installed. 

My initial thought was to stop the array and restart it. Mistake I know. I should have grabbed the smart log. But when I started the array Disk 4 is completely gone now. Any idea what I am looking at here? The drive was active in the array but did not have anything moved on it.

Disk dropped offline, power down, check/replace cables and power back on, if the disk is detected post new diags (note: even if detected it will still be disabled).

After reboot disk is still not showing. Here is new diags.

Now my server is not showing up by its name, I can only access it via the IP address. This is so strange.

And did you replace the cables? If yes disk is likely dead.

I just swapped Sata data cables and its still not showing up. I didn't change the power cable but that drive is the first in the line so it should be getting power since the others are getting power
