Red ball on drive

woke up this morning and found a red ball on drive 5 this morning.  can someone take a look at the attached syslog and maybe see what's going on.  tried to get a smart report, but kept on getting "No such device", pretty sure i was using the correct syntax because i was able to get smart reports for other drives.  i'm using 4.6 (final).  thanks.

Have you moved your system or opened up your machine and moved any of the cables around?

I'm not a syslog expert so I figured I could help out somebody that is with a snap shot and ask that if you moved your machine question.
