Losing drives from array

Over the last few days I am having an issue where my drive 4 and drive 6 are going offline.  I have tried using the GUI xfs repair and also tried the repair for the command line.  It does not report any errors on the drives.  I did once finally get the drives to come back online and ran the Parity Check and all was great for several hours after it completed......Then they both went off line again.  I have replaced the 4Tb drive with another 4 Tb drive that I had not being used.  That drive also appears to be having the same issue.  Any help you can give to fix this issue would be greatly appreciated.

 

I have attached all the logs I thought would be helpful to figure out this issue.

Diags are after rebooting so not much to see, if it happens again post new diags before rebooting.

That makes sense, I didn't think about the log being replaced on reboot.

 

I finally got my array up and the drives are being rebuilt.  If they fail again, before I do anything I will pull the Diag files from the server and post them here.  Thanks for your help.
