Have you tried passing the whole card instead of just the ports? I had a similar problem and passing the whole card fixed the problems.
I tried using these methods for passing through the entire controller and just the webcam.

They didn't work for me. Is that how you did it?
I did the ***GUIDE*** Passthrough Entire PCI USB Controller and New USB Pass Through Method with USB 3.0 at the same time
I took a little bit of work but it wasn't too hard.
