Crap out a file server?  Then simply forget to escape one or more entities when manually editing XML or other formatted config files. 🤣  Don't ask how many hours it took to figure out why my wireless APs and other systems on my LAN weren't getting DHCP leases from pfSense. Uuuugh.  I suppose another way is, don't use characters that need escaping in text fields.
