Array Randomly Stops

I replaced a drive a week or so ago and ever since then, the array stops and I lose connection with the shares.  The web interface loads right  up and I hit 'Start' and everything is fine.  Everything functions normally until a day or so later, it happens again.  I checked out the syslog (attached) and it appears that the array is trying to spin up but then for some reason unmounts all of the drives.  Any thoughts on what could be causing this?

What version of unRAID are you running?

I'm running 4.5.1 and have been for many months with no problems whatsoever.
