I am using the Lime Technology Intel logic board & Promise controllers.. And I have recently (Since 4.1) installed another PATA IDE Card, Not Manufactured by Promise.

 

I then transfered two of my drives to this new card, and these drives are working correctly (Disks 12 & 13), so I am discounting this issue as being a problem with the new card.

 

No matter what I do, Disk 15 shows up as unformatted, even if I take a disk that has been successfully formatted in a different slot. If I try to format it, it never formats.

 

Disk 14 & 15 are on a Promise card.

looks like you might want to try changing some IDE cables, or your hard disks are on the way out

It appears to me that commands sent to disk15-hdl return identical error responses from both disk14-hdk and disk15-hdl.  Commands sent to disk14-hdk seem OK.  So I too would suggest replacing the cable to disk14 and disk15, plus verify that the jumpers on both drives are set correctly.
