Well, it appears that you guys were correct, it did seem to be a usb issue. At ~90% complete rebuild, I started seeing the same read errors that the "bad" drive was experiencing. I've since gotten a new case and am no longer using the external usb enclosure. Drive rebuilt just fine. Oh well, guess since the old drive was fine, I now have another one to add to my array . I appreciate the help, I'll definitely plan to move away from my current usb drive bay. FWIW, I checked that all drives were seated properly, checked cable, rebooted and then attempted to rebuild the failed drive. It immediately began throwing errors, so I ended up replacing the drive and it's currently rebuilding and seems happy again. So apparently, even though there were no SMART errors and the logs indicated a USB issue, I feel like this really was a problem with the drive itself.

I guess I didn't mention this and not sure if it matters, but there are 4 drives in a drive bay, they connect internally sata, with a single usb-c connection back to the computer. I would have thought that they would have all gone offline at the same time if the usb connection was unstable, but maybe that's not necessarily true. Fairly new to Unraid, but today it looks like maybe one of my drives (Disk 4) is having issues and is now disabled. I don't see any SMART errors though. I'm attaching the diagnostics file, but if I was trying to determine the problem myself, what should I be looking for? Is there a flow diagram or list of steps for how to troubleshoot on my own? 6.12.8 Flash Drive Failure - Can't boot
Just wanted to say thanks to @itimpi and @trurl. Ultimately, it appears my thumbdrive was just completely dead. I tried several utilities on both linux and win and couldn't get anything at all read off of it, nor could I reformat as it was marked as read-only. After replacing the thumbdrive and reinstalling from backup, I'm back up and running!

6.12.8 Flash Drive Failure - Can't boot
First time posting, so please be patient... It's unclear if I had some sort of power outage or just a general crash, but was greeted with a startup error and I'm not able to boot my unraid server or get a diagnostic dump. I'm using an i5 Nuc, 16 GB Sandisk flash drive and have been on 6.12.8 for a few days. If I attempt to reboot, about half the time I get a "A bootable device has not been detected" error. The other half of the time unraid attempts to start but fails at the bzfirmware checksum (see attached image). I'm guessing I just need to go buy a new flash drive and attempt a restore? I'm honestly not sure if I backed up after the 6.12.8 upgrade or just before it. I did try the flash drive in different usb ports and tried booting into safe mode. Any help would be greatly appreciated.
