USB Controller Failing During VM Audio Device Interaction

So, I have a USB PCI card which fails when I interact with the connected USB audio device. Everything plugged into it disconnects and doesn't reconnect. This is the related error.

It was previously working fine. Not sure what changed.

 

I have tried different ACS override settings - disabled, downstream, multi-function and also the "iommu=pt" kernel setting.

 

We did have a power drop out recently so i'm not sure how to test for potential damage to the PCI card.

 

Any ideas?
