Diags are being spammed with USB related issues, so difficult to look for anything else, but the disk looks healthy, so most likely a power/connection problem, you should also update Unraid, v6.7 is ancient by now.

    Doesn't look like the flash drive is the problem, the controller keeps resetting, you can try a different one if available.
