Disclaimer: I'm not savvy with Unraid at all, mainly used SpaceInvaderOne videos to set up my server.

Plugins running are Community Applications & Unraid Connect

 

Had a power outage recently and I've had a ton of issues afterwards, mainly that my containers will not run and majority state "database disk image is malformed"  Really not sure what to do and I feel hopeless.

Your NVMe device has issues and needs to be replaced:

 

Jun 16 08:25:13 Moon kernel: critical medium error, dev nvme0n1, sector 0 op 0x1:(WRITE) flags 0x800 phys_seg 0 prio class 0

Thank you Jorge!  I have a 250gb SSD not being used I can use to replace the NVME in the mean time.  Any suggestions on the how I should transfer the contents of the NVME to the SSD?

Try to copy all the data you can manually, you can use your favorite tool like midnight commander or the Dynamix file manager.
