Parity Device Disabled

My Parity device has become disabled.  I have removed the device from the system, removed it from the configuration and then reinstalled it, it immediately goes disabled when the Array is started.  SMART doesn't appear to show any issues, all I can see is that it has 8097 writes, 0 reads and 1024 errors.

 

What's the process for working out how to get it back online, or whether it's a faulty drive?

I should add that stopping the array doesn't work anymore, the only way to get the array offline is to set it not to start automatically on boot and to then reboot the server.  Not sure if that's relevant, but it's an issue that appears to have arisen at the same time.
