Looks more like a power/connection issue with the SSD, replace cables, also having a similar issue with this unassigned disk:

    Ok, I did think I was pushing the limits of my 450w PSU. I'll replace it and change up the sata cables. Thanks! 

     

    *Update* replaced PSU and SATA cables just in case, so far so good. Thanks @JorgeB
