Drive 6 Keeps Red Balling But SMART Tests Pass

Hello!  I am using UnRaid 5.0-rc12a.  Drive 6 in my array keeps showing a red ball.  I have run multiple short and long SMART tests, but they do not reveal any errors.  Since they revealed no errors, I attempted to re-add the drive to the array (using the rebuild approach and not the "Trust My Array" approach).

 

Can someone shed any light as to what the problem may be?  I've attached more details below:
