Unknown reason for instability

My unraid install has been increasingly unstable.  I decided to copy the data off the array to try and backup the non-crucial data that I would not rather re-rip from Blu-ray.  During that process there was a failure.  Attached is the diagnostic.  Does anybody have an idea of what the problem is.
