emhttpd: device /dev/sdx problem getting id

Having an issue re-inserting a previously used disk. Once I hot-swap it back into the array, I get the following error:

emhttpd: device /dev/sdd problem getting id

Unfortunately, this log package is after a reboot and the same error persists. I seem to get this error any time I remove a disk and try to use it again which makes the disk un-usable. Is there any way to "forget" about this drive and re-initialize it?

 

This disk in question is my parity drive. I'd like to add it back into the array but it will not show up in Array Device options or Unassigned Devices.

Drives are connected to a LSI SAS 9211-8i 

 

Any direction would be helpful. I'm new to unraid and this is my first homebuilt server.

I'm able to add this disk back into it's USB enclosure and it works just fine. My concern is that this has happened with another disk recently in the past which I have replaced. I'm not sure what unraid is complaining about here and if there's any way to fix it since it's offline.

Doesn't look related to that, disk is failing to initialize properly, it could be the 3.3v issue, make sure there's no 3.3v on that SATA plug, google "wd 3.3v sata"
