I havent looked at the logs... but kinda sounds like a PSU issue... what hardware are you using?
I agree. Having to use the switch on the PSU sounds like thermal or over current protection.

Got the new drive and finished the swap. Everything went as planned. Anything else I should do now?

Thanks. I'll pick up the new drive tomorrow.

I can get a 4TB drive locally but that would have to become the parity drive. I've had an issue with Amazon and shipping with these disks.

Serial Number: WD-WX22DB0JVF7Z ID# ATTRIBUTE_NAME FLAGS VALUE WORST THRESH FAIL RAW_VALUE 9 Power_On_Hours -O--CK 100 100 000 - 50 197 Current_Pending_Sector -O--CK 196 196 000 - 2167
That is unacceptable for a new disk. Can you return it?
Yes. Can I just replace the drive and allow the data to be rebuilt from parity or should I clone the drive to a new one and then add it to the array?

The new drive arrived. I moved the data from the cloned disk to the drive and copied the data from the 500 GB drive too using Krusader and the videos on Youtube from Space Invader One. All went well. I then started a new config and set the first 3TB drive (the cloned drive) as my parity drive. The sync process has started so I should be back up and running! Huge thanks to @JorgeB and everyone else for getting me back up!!!

FTFY.
Yes, I would request a new drive sent with proper packing. Take photos and record the S/N before you return it.
Amazon will be sending me a replacement. I don't have to ship the other one back before they ship the new one so I can't get the same one back thankfully.

Awesome! I'm traveling for a couple of days, but as soon as I get back I will get started on moving the data to the other new disk.

Are these new disks the same size?
Yes, both 3TB drives. So the final array planned will be 1-3TB for parity and 2-3TB for data (once the 500GB drive is moved to the new drive as well).

OK, now you can move the data to another disk in the array (or UD), like mentioned that disk won't mount in the array because Unraid would complain of an invalid partition.
So I can use the remaining drive I originally bought for parity and move the data to it, then use this disk as my parity drive correct? Otherwise I would have to order another disk as the 2 left in the array don't have enough free space.

Cloned disk won't mount in the array because it's larger than the original, but a filesystem check will still work, let it finish to see if it can find a backup superblock, strange if all are damaged, failing that unassign the disk and try to mount it with UD to see what fs error you get.
The filesystem check ran for hours and never found anything so I'll try UD.

Sorry, that was a stupid question, you're not using the command line, is that filesystem check screenshot from UD?
No, it was from the webgui using maintenance mode.

Please post the full command did you used to run xfs_repair.
I haven't done it. I'm hoping for others with more knowledge to lead me in the right direction. What do you suggest?

No luck on the file check. Do I try to run it again and repair it?
-
ddrescue is finished. here were the results.
Filesystem check is still going, but here is what it looks like so far:

When it finishes should I try to mount it? Or run a file check in maintenance mode first?
Parity drive and data drive giving me trouble

This server has been in this same configuration for YEARS. I don't see myself needing larger disks, but things can always change. In all honesty I could probably get rid of half the data on this server and never miss it (mostly movies that we no longer watch on our HTPC because we can stream them from various services). Instead this server is mostly just a place to backup our other PCs.
