Solutions

  1. Indiana's post in Clean shutdown reboot and usb drive not readable. was marked as the answer   
    It turned out that a bad drive connected to my SAS controller was preventing the USB drive from booting.  I'm not sure why this was happening since the SAS controller shouldn't have been a bootable device.
