Disk offline after 1 error - 6.12.3

Hello all -

I just noticed that my Disk 3 has gone offline. The Main page shows 1 error on this disk, but no SMART errors, and the drive is only 4 months old. What could have caused this and how do I go about recovering? I'd like to avoid swapping the disk out if possible. 

 

Wondering if it might be an HBA issue rather than the disk?

 

I've attached diagnostics.

Disk looks healthy and this:

Aug 21 17:45:47 ana-nas kernel: sd 1:0:5:0: Power-on or device reset occurred

 

Usually means a power/connection problem.

OK, thanks, I'll check the cables. Just to be clear, it could be a problem with either the SATA power, the HBA or the SAS to SATA breakout cable, right? I'm suspecting the SAS breakout cable at the moment.

Got new SAS breakout cables and so far the system seems stable. Thanks
