Crashed server every couple days

Hey all,

Hoping I can get some help with this issue I have been having for a couple weeks. I have recently upgraded my servers MB, RAM and CPU and have been having some stability issues. I have had other crashes previously and replacing the 'bz' files on the USB always fixed it.

Specs:

ProArt X670E (one USB controller passed through)

128GB DDR5 6000 CL30

Ryzen 9 7950X3D

Dual Gigabyte 4090s (one passed through)

Separate PCIe USB controller which I'm not using because of the IOMMU groupings.

Coral TPU for Frigate

Server setup before crash:

Docker: Plex, Frigate, Immich, PostGres, Redis

VM: WIN11 with one MB USB controller, and GPU passed through

Here is the log when it crashed, I nailed the timing down based on my last Frigate recording. I'm not sure if it's a USB issue or not, I am currently using a SanDisk thumb drive (plugged into USB 2.0 port on the case) for now until I get a decent Samsung Bar Plus 3.1 in a few days.

Anyways hoping it isn't hardware issue and just a garbage thumb drive or port.

That looks to me more like a hardware issue, and since memtest is only definitive if it finds errors, if you have multiple sticks try using the server with just one, if the same try with a different one, that will basically rule out bad RAM.

What speeds are you running? With 4 sticks the processor is only rated for 3600, regardless of the rated memory speed.

So after removing the ram sticks from 4 to 1 and back to 2, I had another crash. I have also replaced the Flash drive with a Samsung Bar Plus using a 2.0 port just to rule that out.

The logs looks like a USB error (possibly the Coral TPU) causes a general protection fault which kernel panicked the server? Not sure if I'm reading it correctly

Can't say if that is a hardware or software issue, one thing you can try is to boot the server in safe mode with all docker containers/VMs disabled, let it run as a basic NAS for a few days, if it still crashes it's likely a hardware problem, if it doesn't start turning on the other services one by one.

So I ended up trying beta 3 of Unraid 7 and have had no issues so far...so not sure if the new kernel helped or not. I'm also still getting this error from the Google TPU reset "SuperSpeed USB device number 2 using xhci_hcd" but not as much after getting a high speed USB 3.2 cable...so for now it seems fixed just not sure the fix.

I found that the kernel panic occurs because the 8 bytes of memory are experiencing a use-after-free (UAF) issue.

When the message "xhci_hcd 0000:03:00.0: WARN Successful completion on short TX" appears, the kernel puts the URB into the async_completed list because the TRB completion code is COMP_SUCCESS in the process_bulk_intr_td function. Unfortunately, the USB device copy data via DMA to the transfer_buffer of the URB continue, which has been freed by free_async function.

As a result, when the next kmalloc is called, a UAF error causes the kernel to crash.

This bug only appears with certain USB devices, but unfortunately, I do not have access to those devices.

Perhaps someone who encounters this problem can debug the kernel function process_bulk_intr_td and report the bug to the community.
