[SOLVED] Missing Disk after data rebuild and reboot [5.0.4]

I assembled my server after moving cross country and noticed that disk 6 was showing as "missing" in Device Status.  I put a new drive into my server and selected the newly installed drive in Device Status as disk 6.  Device Status for disk 6 displayed "wrong".

 

I started my array and Data Rebuild began.  Once completed, parity check was successful.  I then rebooted the server and disk 6 displays as 'missing' again in Device Status.

 

Please advise how to proceed.

Your USB drive is not able to save the array state, so it reverts on reboot. Shut down the server, and run a disk check on the USB stick in a windows PC. It will probably prompt to check it when you plug it in.

OK, syslog no longer reports a problem with sda1.  Repeat the Data Rebuild again?

You can. Or select Tools->New Config. Then assign the drives exactly as they are now (record the serial numbers first). Start the array and a parity check will start. There should be zero errors.

 

Make sure to check "Parity is Correct".
