Constant Disk Errors - Please Help

I am getting constant but random disk errors on several disks sending them into error state. At times this has happened while rebuilding the previous disk, other times everything runs smoothly. Seems to be completely random and I don't know what else to do. Diagnostics attached. If anyone knows what to look for I could greatly appreciate it. At the time the diagnostics was downloaded Disk 11 was being rebuilt and 13 became disabled. 

 

I am at the point where I am thinking that a complete wipe and restarting from scratch would be best... Would that solve this?

You have a SAS2LP and those are known to drop disks willy nilly, not surprisingly the two disks currently invalid are connected there, I would start by replacing it with one of the recommended LSI HBA models.

I was afraid that was the issue. The funny thing is I have been using this SAS2LP for a couple of years with zero problems only to have crazy issues the past two week. But I will start looking at an LSI.

It's very common, many users have been using them for years until a hardware or software change, like an Unraid/kernel update, triggers the issues.
