In the Bios there is a setting for IOMMU, it was set to auto.  Changed it to Enabled and everything is working.  I also found this video from SpaceInvaderOne that talks about how changing this setting will also provide better IOMMU performance.

When I start the VM I get the following error.

 

internal error: qemu unexpectedly closed the monitor: 2020-02-28T03:34:28.732937Z qemu-system-x86_64: -device vfio-pci,host=0000:09:00.0,id=hostdev0,bus=pci.0,addr=0x3: vfio 0000:09:00.0: failed to setup container for group 2: Failed to set iommu for container: Operation not permitted

 

VM starts with SSD manually mapped, and Logitech Unify Receiver passed through, but doesn't start when GPU is passed through.
