Hard Drive Keeps Getting Disabled by Unraid

About 4 months ago my Disk 2 began to fail so I purchased a new drive.  Has worked without issue up until about 2 weeks ago when I noticed the drive was disabled in the webgui.  I removed from array and then put back into array so the server could rebuild the drive.  Everything seemed to be fine until a few days ago when this happened again.  Is the drive failing already?  Not sure what to look for or if there is a issue with something else.  Any help is appreciated.

    It won't run a SMART test, It says it's disabled??
