Disk keeps on erroring out despite perfect pre clears and multiple drives

I have been having a problem with disk 3 in my array.  It continuously "errors out" despite being new, having a perfect SMART report, and not having any errors on pre-clear.  I have done this with the same results while also trying multiple drives, power cords, sata ports, and sata cables.  When this happens it also tells me that disk 2 and disc 4 also have errors but after I restart the server the only disc that still has errors in disk 3.

So that controller is built into my motherboard, is there any settings I can tweak either in the BIOS or Unraid to make it work temporarily, my MB is an ASUS Z87WS.  I bought a DELL LSI 9206-16e SAS controller for future drive expansion but I think I need to do some configuration on that, of which I am clueless on, before I can use it. 

 

When I tried to upgrade to version 6.9 I was not able to and had to go back to 6.8.3 and now in the docker tab, under "version" every one of them say "not available".  Could these problems also be linked to this controller?
