Gpu not detected by nvidia driver

I recently added an Nvidia A4000 to my server, and it worked until I rebooted the server and now nvidia-smi and the driver plugin both say no devices found, and the logs show this.

The only advice I've seen from searching is to a) enable legacy boot and b) enable above 4G decoding which... isn't possible? Maybe I'm missing something but my bios says I can only have one or the other so I'm not sure what to do. I'm already doing  legacy boot because I've had issues with UEFI and Unraid before. Any help on this would be greatly appreciated, thanks!

Update: UEFI wasn't working for me in the past, gave it another try with above 4G decoding enabled and getting the exact same error
