If the drive is not seen even after a reboot then it may well have really failed. In terms of testing a disk under Windows you want to go to the manufacturers site and use their test/diagnostic software.

February 9, 201610 yr SMART looks fine, if you can change cables/backplane and retry adding it, if it fails again it's probably a bad disk.
