Communicate new Wireless sensor with smartphones through internet

We have installed new Wireless sensors in company server rooms to trace the temperature readings of server room ,

I have already able to trace the data through Raspberry but need the better solution to communicate sensor reading wireless with smartphone through internet by using apps which perform follow instructions:

- Receive the data from wireless sensor and store the data in database in CSV format and help to trace the previous data by using stored files

- Require dashboard  which display the live streaming of sensor readings in line graph format

- Show the alert notification on smart phones whenever the temperature is not in between the defined range.

Looking for the suggestion, if someone is working or worked on this kind area, Will be much appreciated.

I apologize i am looking for the application which help my new sensor to get connected with smartphone through internet I wasn't aware of sub forum product
