If you can get into your qnap- you should look through the system connection logs.  Update all apps installed on it, firmware, AV, then scan and reboot.

It's just really odd that it's showing as it came from an old IP address I had assigned via my ISP.  Not an internal address.

Is it a large ISP?  Some of the smaller ones use microtik routers that are often vulnerable to the same exploit.  Sometimes the ISP will get exploited and by extension- all the traffic that they serve you is possibly compromised also.  Usually though, they install a proxy or redirection script that allows them to pass through the local device.
