Well after 14 years it had to finally happen. Never seen or wanted too see this message pop up. And checking syslog sure enough I can see multiple instances of the below so I guess my flash drive is toast. At least unRAID has given me fair warning. New Samsung BAR plus on the way to replace the crappy SanDisk UltraFit 🥺

Drive is in a Dell R730xd so cant see it being a power or cable issue. Extended test is about 70% done so will see what it shows up in a few hours.

Correct me if I'm wrong but I believe you would have to replace BOTH parity drives.

Gigabyte GA-890GPA-UD3H - five drives live so far with zero errors
A Sempron 140 is plenty for unRAID unless you plan to run stuff like handbreak on the box too.
