Provisioning CA-signed certificate does not work

Trying to provision a CA-signed certificate. 

When I press the button nothing happens.

How can I resolve this?

Having the same problem 

Unraid 6.11.1

 

I press provisin and it looks like its "working" for 5 sec and then reloads settings page.

 

and it still says 

CA-signed certificate file: Not present

 

from log:
