It's just really odd that it's showing as it came from an old IP address I had assigned via my ISP.  Not an internal address.

Is it a large ISP?  Some of the smaller ones use microtik routers that are often vulnerable to the same exploit.  Sometimes the ISP will get exploited and by extension- all the traffic that they serve you is possibly compromised also.  Usually though, they install a proxy or redirection script that allows them to pass through the local device.
