red dot, dead drive? (and rebuilding new drive?)

I noticed on my server that I have a red dot beside one of my drives and an alert message telling me that the drive was no longer available (I think, this was yesterday). I tried to run a smart test on it but got an error that the drive did not exist so I took it out and put in in a dock on another computer (win) and ran crystaldisk on it and got:

To be totally honest I am not totally sure what all this means, though I am guessing the "Reallocated Sectors Count" part is problematic.

 

My question is how much trouble is this disk in?
