Solutions

  1. Heffe's post in USB Drive Fails After 24h was marked as the answer   
    My server has been running without error for almost a month now. The issue was a bad PCIe to NVMe adapter. (Link to the adapter)
     
    I might have just gotten unlucky with my adapter or maybe they don't work with Unraid, either way, using a proper Dell adapter solved my issue.
