You have no parity and no cache, only 2 data disks in the array and they look fine. Your docker image is ridiculously large but not much used and not related to your problem. It has never been a problem before, but today it was. I was also getting a username/password prompt and none of the accounts I knew were working. That turned out to be Windows being stupid. I had to create an account and password on the unraid server to "log in." Windows 10 and Adblock Plus were the problem all the way around. Link to comment
